Change of Personal Particulars |
As a student, you bear full responsibility to inform the University of any changes to your personal details, particularly your contact information, such as address and phone number, after registration. The contact information provided will be used for all kinds of communication with the University. Please therefore keep your contact information updated via the HKU Portal. The University takes no responsibility for any loss of mail during delivery with inaccurate or outdated address provided. The University assumes that a student’s name is the one which is recorded at the time of admission. Your name should be the same as that shown on your HKID Card or Passport. If you did not submit a copy of your HKID Card during registration, you must provide a copy of your HKID Card to your Faculty Office once it is available so that your HKID number and Chinese name (if any) can be updated in your student record. If your HKID Card record is missing, your Chinese name (if appropriate) will not be shown on your degree certificate and transcript. Requests for amending personal particulars such as name, information of HKID Card or Passport, date of birth, nationality, etc. must be supported by legal documentary evidence. Such requests should be submitted to the Academic Support and Examinations Section for handling. Please refer to the form "Change of Personal Particulars " (http://www.ase.hku.hk/doc/change_of_personal_particulars.pdf) for details. |
