You are required to attend the Health Orientation Talks at Theatre 2, G/F, Meng Wah Complex, according to the following timetable.  Please bring with you the completed Tuberculin Skin Test Questionnaire, Hepatitis B Questionnaire and Chickenpox Questionnaire if applicable.

For Pharmacy and Social Work (Full-time) Undergraduate Students

If you had BCG vaccination before, it is not compulsory for you to attend the Orientation Talk.  You can complete and mail your Tuberculin Skin Test & BCG Vaccination Questionnaire to:

Group B Students Special Arrangement

Students who will not be able to attend the programme on August 25, 2014, they MUST go to T2, G/F, Meng Wah Complex, HKU on September 6, 2014 at 10:00 a.m. with the letter stating the reason for absence.

Students having attended the programme on August 25, 2014 but will not be able to check the test result on August 28, 2014, please go to T2, G/F, Meng Wah Complex, HKU on September 13, 2014 at 9:00 a.m. with the letter stating the reason for absence.
